Newborn and New Patients
Congratulations! We love newborns at FPA. Whether you are adding a member to your family or starting a new one we are here to support you.
You have enough to worry about so we make it simple for your newborn to join our practice. All you need to do is fill out our new patient form and email it to email@example.com or bring it to any of our offices.
To schedule your first visit, just give us a call or a text when you are ready to leave the hospital. Remember to bring your packet from the hospital with you to your appointment.

Whether you are new to the area or looking for a new pediatrician in the area, welcome. Since we are a primary care medical home we do require all documents prior to accepting any new patients into the practice:
1) Completed New Patient Form
2) A complete copy of your prior medical records from previous provider(s), including a full, up-to-date immunization record.
(We require all patients to remain up-to-date on all vaccinations.)
Acquiring these documents is your responsibility. Once you obtain the records, please submit them together with your intake form. You can drop them at any office or email them to firstname.lastname@example.org. When we receive all your records, they will be reviewed for completeness and uploaded into our EMR. At that time you will be notified as accepted new patients and can be scheduled for appointments. We are not able to pre-schedule appointments before officially being accepted.
