Newborn and New Patients
We require all patients to be fully vaccinated per the CDC/AAP schedule. Please review our vaccine policy.
NEWBORNS
Congratulations! We love newborns at FPA. Whether you are adding a member to your family or starting a new one we are here to support you. You have enough to worry about so we make it simple for your newborn to join our practice. All you need to do is fill out our newborn form here: To schedule your first visit, just give us a call or a text when you are ready to leave the hospital. Remember to bring your packet from the hospital with you to your appointment. Feel free to contact us with any questions or if you'd like to come take a look at the practice. |
NEW FAMILIES
Whether you are new to the area or looking for a new pediatrician, welcome.
Prior to accepting any new patients, we require: 1) A complete copy of your entire prior medical records from your previous provider(s), including a full, up-to-date immunization record. A complete record includes all previous visits, labs, vitals, and medications. An immunization record only is not sufficient. 2) A Completed New Patient Form: |
Acquiring these documents is your responsibility. Once you obtain all your records, you may drop them at any office, have them faxed, or email them to [email protected]. After we receive all your records, they will be reviewed for completeness and uploaded into our EMR. At that time you will be notified as accepted new patients and can be scheduled for appointments. We are not able to pre-schedule appointments before officially being accepted. Please also review our vaccine policy.
